KidLavie Waiver & Release of Liability

In consideration of being permitted into KidLavie Family Park and participating in related activities:
Assumption of Risk
I understand that my child/ward and I are voluntarily participating in activities offered by KidLavie, including but not limited to the use of play zones, workshops, the Little SPA, escape rooms, shows, and other facilities.
 I acknowledge that these activities carry inherent risks, including the possibility of personal injury, illness, loss, or property damage. I have explained these risks to my child/ward, and we accept full responsibility for participation.
Release of Liability
On behalf of myself, my child/ward, and our heirs, representatives, and assigns, I release KidLavie, its owners, employees, agents, partners, contractors, and property owners (collectively, “Releasees”) from any and all liability, claims, or causes of action that may arise from participation in activities at KidLavie.

This release applies to all incidents, including but not limited to:

• Use or misuse of facilities and equipment
• Malfunction or breakage of equipment
• Improper maintenance of the premises
• Instruction, supervision, or lack thereof
• Slipping, tripping, or falling on the premises
• Exposure to or contraction of viruses or diseases, including COVID-19

I further grant KidLavie permission to photograph or record me and/or my child/ward during participation, and to use these images or recordings for promotional purposes without limitation.

Safety & Supervision

• Each child must be accompanied and supervised by a parent/guardian at all times unless enrolled in a supervised KidLavie program.
• The indoor playground is a shoe-free zone; socks are required for both children and adults.
• Parents/guardians remain responsible for their child’s behavior and safety while at the park.

Acknowledgment

I certify that I and my child/ward are physically fit to participate in the chosen activities and do not have medical conditions that would prevent safe participation.
 I acknowledge that I have carefully read and fully understand this waiver and release, and that by signing below I voluntarily give up the right to bring any legal action or claim against KidLavie or its affiliates for injuries, damages, or losses related to participation.
